The General Structure of Psychic Suffering La struttura generale della sofferenza psichica

Rigidity or Instability? Rigidità o instabilità?

In the previous essay we left a question open. If the root is the same, why are the outcomes so different? Part of the answer lies in what happens after birth, in the relationship with the caregiver.

Someone who cares for a child does not only give them food, warmth and protection. They also give them the criterion for trusting their own bodily signals. If the child is hungry and cries, and someone responds promptly and consistently, they learn that this signal is reliable and worth listening to. If the response is unpredictable, sometimes it comes and sometimes it does not, they learn that the signal is uncertain. Fotopoulou and Tsakiris titled a 2017 paper Mentalizing homeostasis, precisely to say that the regulation of the body develops within a relationship. In the language of the previous essay, the relationship regulates the precision of internal signals.

Two examples show how this process can deviate in opposite directions. The first is a child with avoidant attachment. In the Strange Situation, the procedure devised by Mary Ainsworth to observe how a child reacts when the mother leaves and then returns, this child shows no distress. They carry on playing, do not cry, and on the mother’s return they ignore her. On the surface they are calm, but the body tells a different story. Heart rate accelerates and cortisol rises, as in a crying child. The signal is there and it is strong, but the child has learned not to give it weight, because expressing it has never brought comfort. The system settles on a fixed value, however inadequate. This is rigidity.

The second example is a child with disorganised attachment. Here the caregiver is both a source of comfort and a source of fear, the paradox described by Main and Hesse. The child does not know whether to approach or withdraw, because the same gesture that should calm them can turn into a threat. No coherent strategy manages to form. The weight given to the signal oscillates between listening and dismissing, without ever settling. This is instability.

Nessuna strategia coerente riesce a formarsi.

The difference between the two branches is therefore one of kind, not of degree. In rigidity the system gives a constant response, however inadequate. In instability it cannot maintain any constant response. In the image of the volume knob, in the first case the knob is stuck, in the second it never stays still. The first system settles on a wrong value, the second cannot settle on any.

A caution. The type of attachment sets the direction, but does not decide on its own. It acts together with the initial vulnerability, and neither is destiny.

To see how real this distinction is, it is worth starting from an everyday phenomenon that we usually do not notice. No one can tickle themselves.
